Eligibility Requirements
- Child must participate in a Medicaid HMO or must have Medicaid Insurance
- Includes children 5 to 17 and adults from 18 and older
- Children at risk for out of home placement,neglect, patterns of criminal behavior or repetitive truancy
- Help transitioning back into the home
- Adults needing assistance with their daily operations and functional living skills
